CPFT patients will take part in a community mental health survey, with forms expected to be distributed in the next few days. This is an annual national mandatory exercise where a large randomly selected sample of service users are asked to complete a survey about their experiences of using Trust services.

The sample will be taken from all service users over the age of 18, open to all community teams, including out-patients, between 1st July 2012 and 30th September 2012. The survey will be conducted by an independent company called Quality Health. Service users selected to take part in the survey will receive a letter from Attila Vegh, Chief Executive, inviting them to complete the survey and a questionnaire.

Participation in the survey is voluntary and patients' feedback will be confidential. Questionnaires are expected to be sent out by Quality Health any time in the next few days. There is a downloadable poster below which staff can print out and display in their service areas to help publicise the event. Please insert the name of the local team manager in the name box on the poster.
