Community heart failure service launches in Huntingdon
Patients living in Huntingdon can be referred to local community heart failure services for the first time. The new service, which launched in January, provides intensive case management for patients with heart failure, including home visits, clinics, telephone support and advice and a cardiac rehabilitation service (group exercise and discussion). The service also manages patients with supportive/palliative care needs due to end stage heart failure. The service will support GP practices to manage patients with the condition.
