We want to involve the people who use our services, and their carers, in the planning of their care.
We use the Care Programme Apporach (CPA) to provide care to people over 16 years with mental health needs. CPA is how we asssess your needs and plan with you the best way of providing treatment and care. More information about CPA is available in the leaflet, attached right.
What does it involve?
- We talk together to assess your health and social care needs
- Together with develop a care plan with details of the help and support that you will receive
- Together we share the responsibility to put the plan into action
- Together we review your care plan regularly to see that it is meeting your needs
- Together we agree any changes that may be needed
- You will have a named care co-ordinator who will work with you
- You will know who to contact and what to do in times of crisis.
Assessing your health and social care needs
We will meet you to discuss and assess your health and social care needs and the care and support you require. With your permission, other people who know you (eg, a close relative or carer) may be asked for their views about your needs. Additionally your GP and other professionals involved in your care will be consulted. We will take account of any cultural, spiritual or ethnicity issues that are important to you.
Your carer will be involved in developing this plan if you wish them to be. You will be given a written copy of your care plan to sign if you are in full agreement.
