Care co-ordinator

My name is Alasdair and I am a care co-ordinator in the Community Mental Health Team for Older People. A care co-ordinator can be from any trained professional background, and is someone who organises and keeps an overview of your needs and which other professionals might need to be involved. I work in a team with people with different professional backgrounds but we all share an expertise in supporting older people with mental health issues.

As a care co-ordinator, if you use our service I will work with you to produce a care plan that is responsive to your needs. You can expect us to talk with you as a person rather than simply looking at you in terms of your diagnosis, if you have one. We support people to be as independent as possible. We try to work with you to resolve the reason you were referred to us and help to arrange help and support where needed.

Depending on the reason you were referred to us it may be a coordinator from a particular profession that will be asked to contact you. However we share a common belief that it is our role to help you be as well as possible.
