Community Mental Health Teams provide support for people who are experiencing mental health problems or distress, through an integrated health and social care service.

How this service can help you

Older people's Community Mental Health Teams (CMHT) provide help and support for people who are experiencing mental health problems or distress. We also help and support carers and families. We aim to promote mental health in the community by helping people experiencing mental health difficulties to stay in the community as long as possible, ensuring their safety, security and wellbeing. We have a range of professionals offering different types of support including: psychiatrists, social workers, psychologists, community psychiatric nurses, and occupational therapists. Care is organised into two pathways; a functional pathway for those experiencing depression, anxiety or other mental health problems, and an organic pathway for people with difficulties stemming from organic change, such as dementia.
