All CPFT services are provided within our innovative, evidence-based care pathways allied with the stepped care model.

Care pathways and rigorous monitoring and governance procedures ensure we care for all patients consistently to the highest standards that meet or exceed NHS guidelines.

When referrals are received, service users are allocated to the most appropriate level of service to meet their needs. Any transfer between care pathways is a clinical decision and seamless without the need for reassessment.

All services provide access to treatment within the NHS 18-week target.

All CPFT services are currently provided to local commissioners in Cambridgeshire and Peterborough. Our specialist services are currently commissioned by PCTs in the east of England and are available across England.

What is a care (patient) pathway?

The planned route that a service user takes from their first contact with a member of the NHS (usually their GP) through referral to the completion of their treatment. Events such as consultations, diagnosis, treatment, medication, assessment, teaching and preparing for discharge can all be mapped on one timeline.

With the help of our service users, carers, staff and commissioners we have redesigned how we deliver care through developing care pathways across our client groups to:

  • Embed the recovery principles into service delivery
  • Provide consistent quality and access across all areas of the Trust
  • Reduce boundaries and bottlenecks between teams
  • Deliver services¬† that are efficient, safe and effective, based on best practice, consistent with national guidance and achieve all relevant targets
  • Focus on the whole client care pathway not just one service
