Community matron

District nursing

Community nursing services include district nurses and community matrons.

District nursing teams assess, plan and provide nursing clinical care to those people who are often housebound due to ill health, either in their own home, or in a care home that does not provide nursing.

They also support:

  • End-of-life care
  • Post-operative care
  • Management of long-term conditions
  • Wound care.

Community Matrons

Community matrons provide advanced clinical assessment and intensive case management (co-ordination of care) for patients who have more than one long term condition such as diabetes, Coronary Heart Disease, arthritis and many more conditions.
 
Community Matrons also support the families of people with long-term conditions and often work closely with hospital consultants, GPs, other nursing staff and social workers to ensure people get the help they need.
