The Joint Emergency Team (JET) is an urgent two or four hour response service that supports people over the age of 65 or those with long-term conditions in their home environment when they become very unwell and need urgent care, but do not need to go to hospital.

The team will carry out an initial assessment and develop a care plan in liaison with the patient’s GP or out-of-hours GP services.

How to refer to JET

Referrals are only accepted from health professionals by calling: 0300 123 9996

Triage hours are 7am – 8pm, Monday – Sunday.
Clinical hours are 7:30am - 8pm Monday – Sunday.

Referrals are accepted from:

  • GPs
  • Paramedics
  • 999 clinical triage
  • 111 clinical triage
  • Health care professionals such as district nurses, mental health professionals, therapists and social care professionals.
