What to expect

On arrival on the unit, service users will be invited to attend a welcome meeting which will include an introduction to the ward by AAU/CRHTT and will be given a Care Team Card which will include details of the primary and secondary nurse and a named HCA for the duration of the admission. Service users will be asked to complete a questionnaire what do you need from us and given a personal recovery file which will include the care plan, wellness recovery plan and daily self assessment sheet.

During your stay

A "you said, we did" board on the unit will display service user/carer suggestions and the actions taken. Nurses and HCAs will be the named lead on dedicated roles. A group programme has been introduced to support the service user in achieving the goals identified in their recovery star (recovery model used to identify strengths) and the areas where further support is needed.

Leaving the ward

A transfer meeting will be held with the service user, their carer, the care coordinator and the transfer team to discuss the next steps. The service user will be asked to complete a how did we do questionnaire so that any unmet needs can be determined and appropriate action taken. The service user will then be transferred to the appropriate stage of care in the 3-3-3 model.
