Neighbourhood teams (NTs) are the physical and mental health care hub of the local community for over 65-year olds and adults requiring community services.

They work closely with GPs, primary care, social care and the third and independent sector to provide joined-up responsive, expert care and treatment.

The 14 NTs operate between 8am and 8pm, 365 days a year, each covering between three and 13 GP practices and populations of 65s and over between 5,700 and 14,000.

Evidence based patient pathways have been developed for a number of health conditions to ensure all NTs manage their patients in the same way and a case management model will identify those high risk people who are most in need of co-ordinated care and at risk of hospital or care home admission. NTs will work in partnership with their integrated care team (ICT) colleagues including
out-of-hours nursing, psychiatry, podiatry, dietetics, mental health therapy services, memory service, speech and language therapy and JET.

NT staff operate within clear governance structures that support patient and staff safety, clinical effectiveness, efficient and productive practice, team and organisational learning, and
staff and team development. An operational policy has been created which will be reviewed every three months.
