The Peterborough Children’s Community Nursing (CCN) team provides a range of services, including support for colleagues, families, children and young people with a variety of conditions within primary/secondary health care teams

How this service can help you

We provide care for children and young people from 0-16 years old who:

  • need acute short-term interventions to enable them to be cared for at home
  • have complex healthcare needs/long-term conditions requiring nursing support / intervention
  • require palliative and end-of-life care

Our care includes:

  • Direct nursing care within the home
  • Care co-ordination
  • Post diagnosis and palliative care and support

We also provide management support to the special school nursing service and health care assistant support in the Child Development Centre.

Facilities available

The team provides services for families who are registered with a Peterborough-based GP practice and some areas of NHS Cambridgeshire (Yaxley, Whittlesey, March, Chatteris and Doddington not covered.

We provide care primarily in families' homes, and also run specific nurse-led clinics and in-reach to some clinics in the local acute hospital.

We no longer run nurse-led clinics.

Referrals are made via GPs and other health and social care professionals working with children and young people within Peterborough and some areas of Cambridgeshire (Yaxley, Whittlesey, March, Chatteris and Doddington not covered).

The service is provided by the Children’s Community Nursing Team of Cambridgeshire and Peterborough NHS Foundation Trust Children’s Business Unit.

The team includes children’s community nurses experienced in the care of sick children, and a secretary.  We are a training organisation and student nurses rotate in the service.
