The Community Cardiac Rehabilitation Team's aim is to improve patient outcomes following acute cardiac event or diagnosis of coronary heart disease.

The Community Cardiac Rehabilitation Team provides support for patients who have experienced an acute, heart-related illness, or who have undergone heart surgery. The service provides domiciliary visits and telephone support following discharge from hospital for patients, delivering individualised, patient centred care.

Holistic assessments include education regarding a heart-healthy lifestyle, identifying risk factors, facilitating lifestyle behaviour change to address risk factors through goal setting and health coaching, help and advice about medications, psychosocial support, signposting/referring patients to the wider multidisciplinary team and progression through to the cardiac rehabilitation exercise/education programmes.

We work closely with our cardiac rehabilitation colleagues at Peterborough City Hospital, Doddington and Wisbech and accept referrals from secondary and tertiary care, GP practices and other health care professionals.
