Community matron

District nursing

Community nursing services include district nurses and community matrons.

District nursing teams assess, plan and provide nursing clinical care to those people who are often housebound due to ill health, either in their own home, or in a care home that does not provide nursing.

They also support:

  • End-of-life care
  • Post-operative care
  • Management of long-term conditions
  • Wound care.

If you have any questions about the services or would like to make a referral you can contact the team on 0330 726 0077.
