Complex Case Management Service

Based in Peterborough and serving both Peterborough and Cambridgeshire. The service is available to children and young people, under the age of 18 years, where:

  • There is a concern about their mental health (this may include neurodevelopmental disorder and/or learning disability)
  • There is a risk of offending (or may have already offended), or high levels of vulnerability to exploitation
  • The young person is moving out of the Secure Estate, Youth Offending Services, Liaison and Diversion Services, SARC, Rape Crisis services, Forensic Child and Adolescent Mental Health Service or Pupil Referral Units; and
  • There are considerable concerns about access to appropriate services due to lack of available service, lack of engagement, or difficulty in accessing services.

What CCM can offer

The Complex Case Management Service will provide support, guidance, and intervention in those complex cases where there is a risk that the child or young person might fall through the gaps between services.

The service provides liaison between agencies and services, and assist with case planning.

The service will also advocate for young people to ensure a smooth provision of mental health support in and out of the health and justice pathway.

In a small number of cases, the service will provide brief specialist assessment and interventions for young people where this would support access to other services.

The service is open to referrals from:

  • Youth Offending Services
  • Secure estate (young offender institutions, secure training centres, and secure children’s homes)
  • Liaison and Diversion Service
  • Sexual Assault Referral Centre
  • Rape Crisis Centres
  • Forensic Child and Adolescent Mental Health Service (FCAMHS)
  • Pupil Referral Units
