How our service can help you
Our service supports patients before and after a cardiac event with the aim of reducing further cardiac events and admission to hospital.
We do this by delivering holistic assessments and individualised, patient centred care, which includes the following:
- Support and education for those at risk of developing cardiac disease.
- Identification of and support to change risk factors associated with heart disease through goal setting and health coaching
- Encourage appropriate lifestyle changes through exercise and education programmes
- Offer advice, education and support with medications and treatments
- Encourage self-management, including symptom management and identification of changes that require further support and intervention
- Signposting and referral to the wider multidisciplinary team, including psychological support services
- Care delivery through domiciliary visits, telephone, and on-line video calls
- Provide support for patients and their families (and palliative care)
- To promote effective communication between primary and secondary care.

To cancel an appointment please call the admin hub on 0330 726 0077.
You will then be put through to our administration department.
Other essential information
- The cardiac rehabilitation programme is tailored to individual need. It is usually for 8 weeks and will include an education and exercise programme, with regular telephone review.

A carer is anyone, including children and adults who looks after a family member, partner or friend who needs help because of their illness, frailty, disability, a mental health problem or an addiction and cannot cope without their support. The care they give is unpaid.
