We are changing the way that care is planned and delivered at Cambridgeshire and Peterborough NHS Foundation Trust.

From 2025, we will be introducing a new co-produce personalised care and support plan for every individual working with our mental health community teams, inpatient services, crisis teams and some of our older peoples services.

We will be telling service users and carers more about this change during the coming months and from November 2025, some service users will start to see this in action.

What is Dialog+

DIALOG+ is a patient-led outcomes tool used in secondary mental health services. It helps you and your care team understand how satisfied you are with different areas of your life and care. You’ll be asked to rate 11 areas using a simple scale:

  1. Your mental health
  2. Your physical health
  3. Your job situation
  4. Your accommodation
  5. Your leisure activities
  6. Your partner/family
  7. Your friendships
  8. Your personal safety
  9. Your medication
  10. The practical help you receive
  11. Your consultations with mental health professionals

These ratings form the basis of a DIALOG+ conversation—a structured, therapeutic discussion between you and your healthcare professional. Together, you’ll focus on the areas that matter most to you and co-produce a personalised care plan that supports your recovery journey.

Why use DIALOG+?

  • It’s about you: DIALOG+ puts your voice at the centre of your care. You lead the conversation, supported by your clinician.
  • It’s proactive and personalised: It helps identify what’s working well and what might need attention, so your care can adapt to your needs.
  • It supports recovery: By tracking changes over time, DIALOG+ helps you and your team understand your progress and plan next steps.
  • It informs service improvement: Your feedback helps shape how care planning is delivered across CPFT mental health services.

When will I be asked to complete DIALOG+?

You may be invited to complete DIALOG+ at different points in your care:

  • At the start: To understand what matters to you and where you're starting from.
  • During your care: Especially if you begin or finish a treatment, experience life changes, or haven’t completed DIALOG+ in a while.
  • When transferring teams: To ensure continuity and clarity in your care.
  • At the end of your care: To reflect on your journey and help improve services for others.

What does my care plan look like?

Your care plan will include:

  • Your DIALOG+ scores and goals
  • Actions agreed with your clinician
  • Notes from your discussions
  • A record of progress over time

You can receive your care plan via SMS, email, or printed copy - whichever suits you best.
