Make a referral

Who is this service for?

The CCPNR accept referrals for children and young people aged between 2-16 years old (or 19 years old if still in education) who have sustained a brain injury after birth. This includes an acquired brain injury as a result of disease, stroke, tumour, accident or other cause.

How do I access this service?

We accept referrals from health and social care teams (e.g. GPs, consultants, paediatricians, social workers), as well as any other agency working with the young person such as case managers and solicitors via personal injury claims.

If the child does not live within Cambridgeshire and Peterborough, and requires an NHS referral, we can apply for funding from their local funding body. Please note, this may take some time to complete and is not always successful.

Please contact us about specific needs and circumstances.

How do I make a referral?

Complete the CCPNR referral form, which can be found in 'Further information' at the bottom of the page, and send to us at ccpnr.mail@nhs.net

See 'Further information' at the bottom of the page for a printable CCPNR leaflet.
