How our service can help you
The heart failure nurse service aims to improve the management of patients with chronic heart failure.
Patients will have early specialist heart failure nurse follow up within two weeks of referral. We provide intensive case management, which includes home visits, telephone support and clinics.
The service also manages patients with palliative care needs at the end stage of heart failure. It also provides support and education for GPs and other healthcare professionals, as well as dedicated education and rehabilitation programmes for patients at home, in the community or hospital. The aim of the service is to:
- To reduce admissions and re-admissions to hospital for worsening or chronic heart failure
- Improve patients understanding of their illness
- Encourage appropriate lifestyle changes through exercise and education
- Ensure appropriate medication and evidience based treatment
- Encourage patients to self-manage, so they can detect possible deterioration early
- Improve quality of life.
