How our service can help you
The Short-Term Adult Reablement Service (STARS) is a small team based in Peterborough offering the reablement model of working with people.
Reablement is a personalised way of supporting you to identify your needs. It is a person-centred approach that aims to encourage and increase independence and wellbeing. It seeks to promote positive changes using your goals. Goal setting is the process of planning for the future and thinking about what you want in life. Goals can be big or small. Reablement is aimed to enable you to learn new skills, re-engage with lost skills and promote confidence in using these to move forward in your life.
We will meet with you to identify what your goals are. This can be in a variety of locations; this may be in your home or in a community setting. Examples of goals that we could support you with:
- Confidence building
- Maintaining your home and tenancy
- Managing your finances / budgeting skills
- Exploring your interests and activities
- Using public transport
- Signposting to other agencies that might be beneficial
- Considering or exploring the idea of voluntary work
- Other goals which are personalised to your own recovery
STAR workers can work with you for around up to eight weeks. The number of appointments offered, and length of each appointment, will be adjusted to your needs on a weekly basis.

Referral Information

You need to be using secondary mental health care and have a named mental health care professional from CPFT. You will also need to live within the Peterborough City Council boundaries. Referrals to be e-mailed by your care co-ordinator or CPFT named professional to CPFTSTARS@cpft.nhs.uk
Is there access and support for patients and visitors with a disability?

Our clinical base is accessible. Please ring the team before your appointment to discuss any requirements. The majority of the work may be completed either in your home or in an environment suitable to the requirements of the work.
