Home Treatment

The CAMHS Home Treatment Team offers a service to young people who are aged 12-18; and their parents/carers whilst they are in the acute phase of mental illness and present with severe impairment in functioning, and/or significant risk or high complexity (for example, complex developmental and attachment trauma, high adversity environments, combined high functioning neurodevelopmental disorder and trauma), where a community CAMH provision would not be sufficient to engage the family and/or young person, contain risk or progress treatment.

The Home Treatment Team offers two pathway options:

Pathway 1:
This pathway is where an in-patient admission may be considered. Where intensive, comprehensive, highly co-ordinated case management is likely to be needed HTT will

  1. Temporarily take over case management and consultant responsibility but will continue to liaise with the community team throughout.
  2. Co-manage assessment and discharge from short stay T4 bed in liaison with the inpatient service.
  3. Facilitate early discharge for those who have been admitted to inpatient care offering either pathway as appropriate.#

Pathway 2:

It will act in addition to and in support of the agreed provision by Tier 3 CAMH services at the time of crisis, or where significant impairment requires an episode of home-based treatment alongside the therapy provided by community services. The task of the Home Treatment Team involvement will be collaboratively agreed by Home Treatment Team/Community Care-co-ordinator and young person/family. This may involve home-based assessment, home-based therapy, family, and other support. It may also involve a second opinion or an episode of structured consultation. Case co-ordination and RMO will remain in the community team.

There is one team for Cambridgeshire and Peterborough which comprises:

  • Service lead
  • Consultant psychiatrist
  • Psychologist
  • Team manager/clinical lead
  • Family therapist
  • Social worker
  • Clinical specialists
  • Senior mental health practitioners
  • Support Time and Recovery workers /Associate Nurse
  • 1 administrator/co-ordinator

The team offers individualised care under both pathways. Therapeutic interventions offered support both young people and parent/carers. These include individual interventions, as well as group interventions such as DBT skills group for young people, DBT parent/carer workshop, Art group, and parents support group.

YOUnited

YOUnited Referral Hub

YOUnited is an exciting, new partnership that started on 1st July 2021 and is provided by Cambridgeshire and Peterborough NHS Foundation Trust (CPFT), Cambridgeshire Community Service NHS Trust (CCS), Centre 33, and Ormiston Families.

The YOUnited Referral Hub is a single point of contact and referral for the partner agencies who provide mental health and emotional wellbeing assessment and support for young people who are registered with a GP in Cambridgeshire and Peterborough. All referrals ask that the young person consents for their information to be shared with the partner agencies with the YOUnited Referral Hub to assist with the seamless transfer of care where necessary.

Professional referrals are accepted for children and young people with mental health or emotional wellbeing needs up to the age of 17 years (or 18 years for those referred with a suspected eating disorder or with a neurodevelopmental diagnosis) (or 25 years for referrals to Centre 33).

This integrated referral hub triages and screens all referrals based on risk, clinical mental health needs (NICE guidance thresholds for CAMHS) and impact on functioning. The referrals are moved through the hub by using the National i-Thrive model for pathways of support:

  • CAMH specialist support, (“getting risk support” or “getting more help”)
  • Support from partner agencies – CCS, Centre 33 or Ormiston Families (“getting help”)
  • Signposting to external agencies / statutory organisations (“getting help”)
  • Giving advice and guidance (“getting advice”).

YOunited referral hub operates during working hours (9am – 5pm, Monday to Friday) The main office is based in Huntingdon, Cambridgeshire with a mixed pattern of both office and remote / home working.

The YOUnited clinicians or practitioners use SystmOne (electronic records system) to process and screen referrals. Screening referrals might mean contacting the family, referrer or other professionals involved with the young person by phone or a virtual conversation. We collaborate with local statutory organisations (Early Help Hub, Children’s Social Care and schools) to seek relevant information and decide on support that will best meet the needs of the young person and their family. If a referral is being signposted to an external agency / organisation, a letter is with this information is sent to the referrer, GP and the family (and young person where relevant).
