We are a team of dedicated, experienced professionals working together to care for and support people living with diabetes. 

Diabetes is a complex condition which is affected by, and can affect, almost all daily activity. Almost every day to day decision affects blood glucose levels, whether thats eating, exercising or taking medication. Therefore people living with diabetes are under constant pressure to manage their condition and this can be a mammoth task.

People living with diabetes require knowledge and skills to enable them to understand the effects of lifestyle on their diabetes and vice versa, as well as how they can manipulate their treatment to enable them to lead the lifestyle of their choice while maintaining stable blood glucose control. They also need information on the consequences of poor control of their diabetes so they can make informed choices in setting appropriate personal goals for the management of their diabetes.

The role of the diabetes health care professional is to provide support to people with the condition to enable them to develop realistic short term and long term management goals, as well as to help them to acquire the knowledge and skills necessary to achieve those goals.

The team offers care, support and education to make these goals as achievable as possible. It exists to give people best chance of living a life free of worry and stress and to be as physically and mentally healthy as possible. Below are just some of the things our service can do to help you:

  • Avoiding complications

  • Promoting self-management
  • Reducing hospital admissions
  • Social and psychological support

Referral Information

  • Self referral accepted if you have type 1 diabetes or a young adult up to the age of 25 years.

    Self referral is also accepted for DESMOND type 2 education programme.

Where to park and are there parking charges?

  • Free parking at various clinic site locations.

Is there access and support for patients and visitors with a disability?

  • Yes, our clinical bases are accessible. Please ring the team before your appointment to discuss any requirements.

How to cancel your appointment

  • Please call the admin hub.

Carers Information

  • A carer is anyone, including children and adults who looks after a family member, partner or friend who needs help because of their illness, frailty, disability, a mental health problem or an addiction and cannot cope without their support. The care they give is unpaid.

    At Cambridgeshire and Peterborough NHS Foundation Trust, we value the often life-long support carers provide and recognise them as equal care partners. We want to offer as much guidance and reassurance as possible, to help you in your caring role.
