Referral Information

  • Parent/Patient and Preschool referral: is found on www.cpft.nhs.uk/training/referrals-and-drop-ins.htm named the (EAS) Early Advice Session Registration Form. Primary /Senior School Referral: SENCO and patch Therapist to complete an RFI form and SENCO to email this to cpm-tr.SLTmailforparents@nhs.net along with the parents consent form.

    FOR PROFESSIONALS ONLY: Health Visitor - ask parents to complete EAS. If pre-schoolers are currently under EHA necessary referrals will be made by the EHA team. GP/Other Hospital dept to email cpm-tr.SLTmailforparents@nhs.net for Complex needs only.
