How our service can help you
The team provides short-term (up to four weeks) care and therapy support to patients in their own homes, or in intermediate care health beds in care homes, to support discharge from hospital or to prevent admission to hospital.
This service includes full assessment of your care and rehabilitation needs to support your recovery and increasing your independence following acute illness. You will work with our nurses, physiotherapists, and occupational therapists to achieve your rehabilitation goals.
If you have long-term therapy goals and care needs at the end of your stay with our service, the team will work with you to refer you onto services that will continue to support and work with you, such as community therapy and adult social care.

We do not have clinical rooms. We visit patients in their own homes and complete care and therapy.
