Intermediate Care Unit

How our service can help you

The Intermediate Care Unit is a physical health inpatient rehabilitation facility that is nurse led, supported by GPs and a consultant geriatrician.

The unit's multi-disciplinary team assesses patients to identify treatment goals and agree a provisional length of stay when they are admitted. These goals are then reviewed and progress charted each week. Discharge planning starts on admission so that complex issues are addressed.

Patients and their families are encouraged to take an active role in planning and working together to achieve the best outcome possible. If necessary, referrals are made to other agencies or professionals to support patients when they are discharged.

  • Business Hours/Visiting Hours: The unit is open for visiting seven days a week between 13:30-17:00 and 18:00-20:00.

Referral Information

  • No referrals from public. Referrals are via hospital discharge planning, Joint Emergency Team (JET), and integrated community services.

Where to park and are there parking charges?

  • Parking provided by NHS Property Services at City Care Centre - charges normally applied, controlled by NHS Property Services.

Is there access and support for patients and visitors with a disability?

How to cancel your appointment

  • We provide inpatient services, not outpatient.

  • A carer is anyone, including children and adults who looks after a family member, partner or friend who needs help because of their illness, frailty, disability, a mental health problem or an addiction and cannot cope without their support. The care they give is unpaid.
