How our service can help you

Trafford Ward is a  physical health inpatient rehabilitation facility that is nurse led, supported by GPs and a consultant geriatrician.

This wards also provides end-of-life care.

The ward’s multi-disciplinary team assesses patients to identify treatment goals and agree a provisional length of stay when they are admitted. These goals are then reviewed and progress charted each week. Discharge planning starts on admission so that complex issues are addressed.

Patients and their families are encouraged to take an active role in planning and working together to achieve the best outcome possible. If necessary, referrals are made to other agencies or professionals to support patients when they are discharged.

Our end-of-life care patients and their families are supported and cared for by ward nurses, specialist nurses and a consultant geriatrician, who will assess, plan and implement care on an individual basis.

  • Referrals are accepted from health/social care professionals to a single point of access on 01353 652135. They are reviewed via the triage team at the admin hub.
