The Peterborough Children in Care Team carry out statutory health assessments of all children and young people who are looked after by the Local Authority. We offer consultation, advice and support to promote the health and well-being of looked after children and young people (up to 18 years old). The team is made up of consultant paediatricians, specialist nurses and administrators.

We provide an initial health assessment on all children and young people in care in line with the statutory guidance of the Children Act 2004. After an assessment by the designated paediatrician and specialist nurse, we work with children and young people, carers and professionals involved in their care to design a tailored health plan.

Our service includes:

• Assessment and health plans
• Consultation, advice and support for children, young people and their carers to optimise health and well being
• Partnership working with education services, social care, the police and voluntary agencies to provide multi-agency liaison and training

We see children under five years old every six months and school-age children at least annually. We mainly see children and young people at the City Care Centre, Peterborough, but also see them in other places such as within the local authority, their school or home, depending on their individual needs.

Who will you see at your health assessment?
You will be seen by a nurse or a doctor.

What happens at your health assessment?
You will be invited to attend your health assessment with your carer. However, you will also be given the option to talk to us on your own. Your health assessment will be all about you. It’s your chance to talk about your health and how you feel We will measure your height and weight.

How often will you be seen by our team?
A child between 0-5 years will be seen by one of us every six months.
If you are aged 6-18 years you will be seen by us once every year.
If you have any health issues that you need help with, you may be seen more often. If this is the case, we will tell you about this at your appointment. We won’t share your information with anyone else, unless you say we can. Sometimes we need to let other health staff know about you, but we will always ask you first if this is OK.

What happens after your health assessment - what do we do with your information?
After your appointment with us we will produce a Health Action Plan. This will include things like the last time you saw a dentist or optician, your height and weight. We will include any health actions that you may need for example, If you need to see your doctor for something we have talked about it at your health assessment. Your health action plan will be sent by us to your social worker. If you would like to see a copy of your plan, please contact your social worker.
