How our service can help you
JET is an urgent two- to four-hour admission avoidance response service that supports people over the age of 18 years in their own home when they become acutely unwell and need urgent care, but do not need to immediately go to hospital.
The team consists of paramedics, registered nurses and two therapists. Following a comprehensive clinical assessment, a plan of care will be developed along with the patient, carers, and family.
If required, senior clinical support will come from the GP out-of-hours service and the Urgent and Emergency Care Hub (UEC).

Referral Information

Referrals are via telephone through The Urgent and Emergency Care Hub on 0333 332 6800 option 2.
For internal CPFT referrals, call the CPFT Admin Hub on 0330 726 0077.
Service criteria
- Any patient over 18 years old who is house-bound and has a long-term condition, with an onset of acute needs.
- Patients registered with Cambridgeshire and Peterborough GP.
- Patients living in their usual place of residence, inclusive of nursing homes, care homes and residential homes.
How we can help
- Falls reduction in mobility.
- Acute exacerbation of chronic health issues such as COPD and Parkinson's disease.
- Simple chest infections and urinary tract infections (not requiring IVs currently).
- Acute wound care (not requiring suturing).
- Urgent basic equipment needs – assessment and provision.
- Access to up to two weeks' intermediate care in patient's own home following assessment by a JET practitioner.
- Access to step-up beds in community hospitals.
- Work with mental health crisis teams providing physical assessments ruling out organic causes of mental health illness.
- Bridging healthcare needs for patients at home in partnership with Discharge to Assess, pathway 1 (intermediate care).
Where we cannot help
- Acute medical emergencies such as CVA, MI, GI bleed, acute abdomens.
- Seizures or head injury on warfarin fractures.
- Falls where a referral to the JET service is inappropriate (for example, head injuries on anticoagulant therapy).
- Intravenous fluids or antibiotics.
- Social care crisis (with no health needs).
- Acute mental health needs including drug and alcohol misuse.

There is access at all bases with flat thresholds and lifts.

- Patients cannot self refer.

A carer is anyone, including children and adults who looks after a family member, partner or friend who needs help because of their illness, frailty, disability, a mental health problem or an addiction and cannot cope without their support. The care they give is unpaid.
